We do not pass any individual's consent to receive SMS from Zabella Insurance Agency to any third party. We do not share phone numbers with third parties for the purpose of allowing those third parties to send SMS.

2. Regulators and Law Enforcement: As required by law.
3. Health Providers: To facilitate care and treatment.
4. Healthcare.Gov: To help you enroll in a Marketplace Qualified Health Plan (QHP). Any PII we collect is used to help you enroll in a Marketplace Qualified Health Plan (QHP) (and other related products you select, if applicable). If you choose to give us PII, this information will only be shared with CMS and the insurer at the time of enrollment. CMS will maintain this information in the federal System of Records.
PII is used or disclosed only under the following circumstances:
1) to Zabella – TCR-RC Rejection Issues & Recommendations compare insurance plans based on costs, benefits, and other important
features;
2) to determine eligibility for health coverage and costsharing reductions through healthcare.gov or an EDE enrollment portal;
3) to choose a plan; and
4) to enroll in coverage.
Providing your PII is voluntary. If you choose not to provide us with the PII requested, or not to respond to certain required HealthCare.gov questions, we will not be able to help you enroll in a QHP through the Marketplace.
